# Rybelsus (oral semaglutide)

> Rybelsus is authorised in the EU for type 2 diabetes only — how it works, dosing on an empty stomach, and why weight loss use is off-label.

Rybelsus is a tablet form of semaglutide. It is licensed in the European
Union, but only for one thing: controlling blood sugar in adults with type 2
diabetes. It is not licensed in the EU as a weight-loss medicine.

<Warning>
  People taking Rybelsus for diabetes often lose some weight as a side effect,
  and that is where its reputation as a weight-loss pill comes from. Prescribing
  it purely to lose weight is **off-label** — used outside what EU regulators
  have assessed and approved — so the EU-assessed benefit-risk balance for that
  use has not been established.

## What it is

Rybelsus contains oral semaglutide, a copy of the gut hormone GLP-1 taken as a
daily tablet rather than an injection. It has been authorised for use in the
EU since 3 April 2020, for adults with type 2 diabetes. Any place it takes in
a weight-management pathway is an individual doctor-led decision and may be
off-label.

<Note>
  A separate, higher-dose oral semaglutide tablet (25 mg) was authorised by the
  European Commission for weight management in **July 2026**. That is a
  different product from Rybelsus — if you're looking for a semaglutide tablet
  specifically for weight, a doctor should decide which one, if either, applies
  to you.

## How it works

Semaglutide activates GLP-1 receptors, supporting glucose-dependent insulin
release, reducing glucagon, and slowing stomach emptying. Appetite may also
decrease as a result.

## What the studies reported

The pivotal trial, **PIONEER 4**, was a type 2 diabetes trial whose main
purpose was to measure blood-sugar control
([ClinicalTrials.gov · NCT02863419](https://clinicaltrials.gov/study/NCT02863419)).
Weight change was a secondary measurement in people with diabetes — the trial
was not a test of Rybelsus as a weight-loss treatment, and Rybelsus has no EU
weight-management indication. A readout is not the same as peer-reviewed
publication; results here are grouped by source and evidence grade so journal
data and manufacturer announcements cannot be confused.

## How to take it

    First thing in the morning, at least 8 hours after your last meal.
    Swallow the tablet whole with a sip of water — no more than 120 ml. Do
    not split, crush or chew it.

    other medications, so the tablet is absorbed properly.

## What has been reported

| Diarrhoea | Very common — around 10% of people taking semaglutide |
| Vomiting  | Around 7% of people taking semaglutide                |

Effects are usually mild to moderate and settle over time, most often in the
first weeks. Nausea often improves within 1–2 weeks; smaller meals eaten
slowly, and avoiding fatty or fried food, can help.

  Store below 25°C in the original packaging, protected from light and
  moisture, and do not use it after the expiry date.

## Managing side effects

Nausea is very common in the first weeks. Eating smaller meals, avoiding fatty
or fried foods, and eating slowly can help. Drinking plenty of water (6 to 8
glasses a day) and including fibre-rich foods like vegetables and whole grains
may also help. Some people feel tired in the first weeks, but this usually
passes.

    alcohol. However, alcohol can affect blood-sugar control, and the
    reviewing doctor discusses this as part of the consultation.

    Semaglutide begins to act from the first dose, but appetite changes may
    not be noticeable for 2 to 4 weeks. Weight loss is assessed at 3 months,

## Who it is not suitable for

* You are pregnant, breastfeeding or planning pregnancy
* A history of serious semaglutide reaction, or an unresolved pancreatic or
  gastrointestinal concern, makes it unsuitable

  Contact your doctor for severe abdominal pain that does not go away,
  persistent vomiting, signs of an allergic reaction (rash, itching, swelling),
  or any symptom that worries you.
